Exosomes are nanoscale vesicles released by cells, typically 30 to 150 nanometres, that carry proteins, lipids and genetic material between cells. They are a signalling system, not a cell.
Because exosome preparations contain no living cells they are described as cell-free. That is a real distinction from stem cell therapy, and it changes both the biology and the regulatory picture.
The evidence base is immature. Most studies are small, single arm, non randomised and rated at moderate to high risk of bias. Long term data does not yet exist.
Documented harm exists. A 2025 case series reported persistent inflammatory nodules and residual scarring after intradermal injection of poorly characterised exosome formulations administered outside medical supervision.
Delivery route matters enormously. Topical application after a procedure, intradermal injection and intravenous administration are three different clinical and regulatory propositions.

The Science
What Exosomes Are, and Why They Attract So Much Attention
Cells communicate constantly. One of the ways they do it is by releasing small membrane bound packages into the surrounding fluid. Exosomes are one class of these packages. The exosome does not do the work itself, it delivers instructions to cells that already live in the tissue. This is why the literature describes exosomes as mediators of paracrine signalling.

Not all extracellular vesicles are exosomes. The broader category also includes microvesicles and apoptotic bodies, which differ in size, origin and behaviour. Exosomes used in practice are most often derived from mesenchymal stem cells, though published studies also use platelet derived, adipose derived, placental, bovine milk, bacterial and plant derived vesicles. These are not interchangeable. The cell of origin substantially determines what the vesicle carries and how it behaves, which is one reason product comparisons are difficult for patients to make.

Emirate level authorities, including the Dubai Health Authority, license the facility and the practitioner. Three things follow.

Safety, Expectations & Honest Messaging
In controlled clinical settings, reported adverse events are predominantly mild and transient: erythema, swelling, pinpoint bleeding, dryness and short lived discomfort, largely attributable to the delivery procedure rather than to the vesicles themselves. The significant exception matters. A 2025 case series in the Journal of Cosmetic Dermatology documented four adult women who developed persistent inflammatory reactions after intradermal injection of exosome formulations of unspecified origin, administered outside proper medical supervision. Reported findings included prolonged erythema, nodules, suspected granulomatous inflammation, incomplete resolution and residual scarring. The lesson is specific rather than general: the harm was associated with unregulated products, injected intradermally, without proper clinical governance. Product provenance and delivery route are not administrative details, they are the risk.
